Accommodations Card

So this is probably a really dumb question but I have a question about the accommodations card, so I'm saying the following.  Please let me know if this is all correct?


 


 For your convenience, a block of rooms have been reserved at the following hotels. Please refer to the _/_ wedding when making your reservation


 


So should I put a comma after convenience and then a period at the end after reservation? 


Also should it say a block of rooms HAVE been reserved or a block of rooms HAS been reserved?

has - you are referring to the "block" not the "rooms"

Yes on the period after reservation as well.
