Ultrasound and Clomid

As some of you ladies know, I am on my second round of Clomid.  My doctor's nurse just called me and wants me to come in on CD 12 to do an ultrasound.  This is the first time I have had an ultrasound done while on Clomid, so I was interested to hear from you ladies what this ultrasound will tell me.

I am very excited that she will be monitoring me this way, since last time all we did was the CD 21 blood test.

Thanks for any information you can give me!

Yay!  That's awesome!  I know you were saying before how you wished your doctor would do these!  I actually have my first one today (CD 9 since I usually O around CD12) in about two hours so I'll fill you in on how it was afterward.  But my understanding is that they will look at your ovaries (using a vaginal wand) to see if any/how many follicles are growing and they will measure them to see if you are close to ovulating.  I know they can also check the lining of your uterus to see if it's thinned out too much from the Clomid.  Not sure if that's something they will automatically do today but it's something I'm going to ask about.  I'm excited for you!! :)

Okay so I had the u/s appointment...not bad at all.  A bit depressing to be sitting in the waiting room with nothing but very pregnant women waiting for their prenatal u/s appointments but that's a whole other issue!! :)

So today on CD9, I have three follicles - one on my right at 12 mm and two on my left at 13 mm.  They usually grow about 2 mm a day so based on that, she projected that I would ovulate on Monday, CD12, which is my norm (sometimes 11 sometimes 13).  At least one follicle would be at least 19 mm by then, if not more than one.

My uterine lining was only 5 mm.  She said at the time of ovulation, they like to see 10 mm but I've read online that RE's say anywhere from 6-12 is okay.  The lining apparently grows 1 mm per day so by O for me, my lining would theoretically be around 8 mm or so.  Not sure if that's too thin or what....who knows.

Now, I'm taking what this u/s tech said with a grain of salt because when she told me she thought I would ovulate on CD12, she then followed it up by telling me that CD12, 13, and 14 were the days to have sex.  Um...doesn't the egg only live 12-24 hours??  She said if I wanted, we could start DTD on the evening of CD11 but that 12-14 were more important.  Uh huh.  Also, when I told her Clomid has really dried up my CM, she seemed suprised and actually made the comment that she thought it was supposed to do the opposite.  Um, no.  Clomid blocks estrogen production and estrogen is needed to produce CM.  So because of all this, I'm not stressing too much about the u/s with her.  Thank GOD I'm moving on to an RE for next cycle!!  Well, hopefully I won't need to but if I do, then thank GOD! Ha! :)

Oh, and when I went in for my Clomid check last week, the nurse practitioner said I would get an u/s on CD9and CD12 to make sure we really tracked O.  But the u/s tech today didn't want me to come in again before O.  I'm going in on CD14, AFTER O to see "what's left" and check my lining thickness.  That seems a bit pointless but whatever.  :)

beatie sorry to bump in here but they say sperm can live up to 4 days. (I didnt believe it) For my cycle that I got my BFP I O'D Januarary 24th on a Monday and the only days we DTD were the 20th and the 27th. So even if the egg only lives 12 to 24 hours the sperm can be there "waiting"for it. Worked for me! Good luck ladies!

Oh I understand that sperm can live for a few days in ideal conditions (I don't have ideal conditions because I produce little if any CM).  But that's not was she was saying to me.  She said that if I O on CD 12, then I should DTD on CD 12, 13, and 14.  It would make sense if she said DTD the three days BEFORE O but she told me to DTD three days AFTER O.  If I wait to DTD until the days after O and don't DTD at all before O like she suggested, that's going to be too late.

Oh sorry I misunderstood! Yeah that doesn't make any sense to why she would have you start at CD 12 specialty if your CM is low. I hope you get your bfp soon or like you said good thing you are moving on to a RE. I hope this is your cycle! I stalk this forum a lot hoping you ladies get that double line.

I'm so annoyed, at this point, when I go to see a doctor -- pay a co-pay, pay a monthly insurance fee, and pay for all the extras (blood work, meds, etc) -- only to have a medical professional give me completely wrong information!!  What's up with that?  I say keep your month shut unless you are informed and have a correct answer!  ...Sorry, just had to vent after I saw your post, beatie.  If you didn't know any better you might have listened to the wrong information, which, I bet, is what is happening all of the time to other women!  Anyway, I'm glad you got your u/s!  Your follicles are looking good!

ccia -- I really glad you are finally getting a shot at this too!  It can be a great tool to assess what's going on.  Today I had a u/s on CD 11 to see if my hormones where doing their magic... the doctor was able to tell me immediately that my uterus looked good, but he did not mention a number... I wish he did, but oh well!  Make sure to ask about that when you go in....  Also, he looked at both ovaries and told me that I have several maturing follicles, but doesn't know which one(s) will become predominant in the next couple days.  He rattled off some numbers to the nurse, who wrote them down in my chart, but (again) he didn't explain what they meant... I got the jist of how I was progressing (I think my biggest follicle is just at 12mm), but unfortunately didn't butt in and ask more questions!  Make sure to do that if they skim over something... I wish I did!  Then the nurse told me to come back Sunday morning.  By then, she said, my ovaries should be ready for the trigger shot, and then Monday and Tuesday (or just Tuesday if the timing looks right) I will get my IUI's!!!!

P.S. Thankfully vaginal u/s do not hurt one bit... just maybe messy!  And they are really quick too!

hi ladies! great info! my RE did the same for all of my US's. She measured my uterine lining and checked to see if I had any follicles developing. The first one there were only small little things, the second one, a follicle size of 8---those two were on clomid. My third one, I had a follicle size of 18 and that was on femara.

Beatie, I'm curious, are they going to give you the Hcg trigger shot to help stimulte O? That might be why you were told to DTD on CD 12, 13, 14?

No, no trigger yet.  I'm only with my ob/gyn and I think that's far beyond their knowledge.  I already O on my own unmedicated and since I've been charting for 13 cycles, I know that most likely I will O on or around day 12 (her estimate only reaffirmed that guess for me).  She told me those days because by her calculation of how much follicles grow each day until they are big enough to release, she estimated that O would be CD12.  So when she said to DTD on 12, 13, and 14 I knew she wasn't totally sure of what she was saying...

beatie - sorry that nurse gave all the wrong information to you - I would just go with what you know, since that is what I know to be right too...Clomid not drying you up??? I can't believe she said it's the opposite!!

The good thing is my actual doctor is the one who performs the vaginal ultrasounds...I don't know if she does them for all of her patients, but I LOVE it because I can ask her questions and she can respond right then and there.  Thanks for all of the advice on what to ask for - I really appreciate it!!! :)
